What the HSSIB staff fatigue report means for your rota.
The Health Services Safety Investigation Body published its investigation into staff fatigue and patient safety in April 2025. Here is what it found and what it means you need to do next.
The HSSIB investigation found that fatigue is directly and indirectly linked to preventable patient harm. The findings are not new to anyone working in healthcare. What is new is that the HSSIB has now made an explicit recommendation: organisations need systems to measure, monitor, and manage rota fatigue. Most do not have one.
The CQC is now citing rota fatigue in inspection reports. Under Regulation 18 (Staffing), providers must have enough suitably qualified, competent staff to meet people’s needs safely. Rota overwork is the most visible and measurable form of non-compliance. A provider that cannot show they are actively scoring and monitoring fatigue risk has no defence when things go wrong.
